Identity Theft Complaint
To file an ID Theft complaint with the Attorney General's Office, follow these simple steps:
1. Download the ID Theft complaint form
2. Complete the form by typing in the interactive fields within the pdf
3. Once completed, you must print, sign and mail the form, along with the all relevant supporting documentation to the following address:

*Please send copies of your supporting documenation so you may retain the originals. You are also advised to keep a copy of your complaint for your records.
