Complaint Form

To file an identity theft complaint with the Attorney General's Office, follow these simple steps:

  1. Download the Identity Theft complaint form
  2. Complete the form by typing in the interactive fields within the pdf
  3. Once completed, you must print, sign and mail the form, along with the all relevant supporting documentation to the following address: 

    Consumer Protection Division
    Office of the Indiana Attorney General
    302 W. Washington St., 5th Floor
    Indianapolis, IN 46204

 

*Please send copies of your supporting documentation and retain the originals. You are also advised to keep a copy of your complaint for your records.
