To file an identity theft complaint with the Attorney General's Office, follow these simple steps:
- Download the Identity Theft complaint form
- Complete the form by typing in the interactive fields within the pdf
- Once completed, you must print, sign and mail the form, along with the all relevant supporting documentation to the following address:

*Please send copies of your supporting documentation and retain the originals. You are also advised to keep a copy of your complaint for your records.
