Salvage title applications are processed by BMV Central Office Title Processing to improve the security and efficiency of these transactions. Prior to submitting each application verify all required information is included. Call the BMV Contact Center at 888-692-6841 with any questions.

A salvage title is required for motor vehicles manufactured within the last seven (7) model years. When requested, a salvage title may be issued for a vehicle over seven (7) model years old.

When submitting the paperwork, include the following:

  • Completed and signed Application for Certificate of Title – State Form 44049
  • Salvage Title Affidavit - State Form 49891
  • Certificate of Title
  • Physical Inspection of a Vehicle – State Form 39530.  Required for out-of-state vehicles
  • Proof of Indiana residency. Examples include a utility bill dated within sixty (60) days, United States Postal Service change of address confirmation, W-2, etc.
  • $4.00 title application fee payable by credit card (MasterCard or Visa), check, electronic check, or money order. A $10.00 delinquent fee will be assessed on packets received 31 days after the date of purchase, if applicable.
