Below is the link to the Application for Title IV-D Child Support Services. Printing this form and filling it out completely prior to going to your county Title IV-D prosecutor's office will save time for all. There is no fee charged to families who receive Temporary Aid for Needy Families (TANF) or Medicaid. A nonrefundable onetime fee of $25.00 payable in the form of a money order is required to start child support enforcement services for families not receiving TANF or Medicaid.
