National Fire Academy

Short Form Application

  • Use this application if your course code begins with the following letters: F, H, W or Y. FEMA Form 119-25-2 is also used for Q133, H134, and any conferences held on the NFA Campus.

General Admissions Application

  • Use this application if your course code begins with the following letters: C, N, O, P, R or T.

FEMA Student Identification Number

  • Required for ALL National Fire Academy course applications.  Social Security Numbers will not be accepted.
  • Visit to apply for your SID number.
