ID Required
Effective Jan. 2, 2007, photo identification is required for all transactions at any Indiana Department of Revenue office. Examples of transactions include, but not limited to inquires about:
- Tax preparation
- Individual-account information
- Pick-up refund check
- Business-account information
- Business-tax application
The Department will accept the following forms of photo identification:
- State Issued Driver’s License
- State Issued Identification Card
- U.S. Passport
- Military Identification
