Deaf & Hard of Hearing Services (DHHS), a program of the Bureau of Rehabilitation Services (BRS), Division of Disability & Rehabilitative Services (DDRS) provides assistance to identify and find resources to meet the needs of deaf and hard of hearing individuals and their families, throughout the State of Indiana.
Before sending the State Agency Communication form, please perform the following steps.
Please note: Once your file is saved on your computer, you will not need to retrieve the form from the DHHS website, but can open the file you have saved and simply modify the form for a new request (change the name of the document, ie. Smith 1.15.2011).
