Check-List Before You Turn in the Application
- Double-check the children's social security numbers and birth dates
- Make sure as many questions are answered as possible
- Attach copies of documents that show proof of your income
- Attach proof of citizenship
- If necessary (see What Other Things Do You Need), attach proof of private insurance, immigration status (if a child is not a U.S. Citizen), and pregnancy (or the name of the doctor who can verify pregnancy). (Note, documents may not be able to be returned if mailed-in)
