Eligibility requirements for a Cemetery Heritage sign can be found in the Cemetery Heritage Sign Application. 

Eligible applicants should complete and return an application to Cemetery Registry Coordinator; Division of Historic Preservation and Archaeology, DNR; 402 W. Washington St, Room W274; Indianapolis, IN 46204.

IHB has established the following deadlines for Cemetery Heritage Sign applications.  Applications received by the deadline will be ordered from the approved vendor as a lot and delivered per the schedule below:

 June 15, 2012     August 1, 2012 October 31, 2012
 February 15, 2013 April 1, 2013 May 30, 2013

 *IHB will send an invoice and text approval form after the application has been received and processed.  The cost of the sign must be prepaid and the approval form returned by this date or the application will be moved to the next deadline and delivery will be delayed.
