- Payment Information:
- Payments under the Agreed Order shall be made by certified check or cashier's check made payable to the "Four County Landfill Site OU2 Trust". Do not send the check to IDEM. The check, and a transmittal letter accompanying the check, shall reference the name and address of the party making payment, the Site name, and the IDEM Site Identification Number 00000134 and shall be sent to:

- Payments under the Agreed Order shall be made by certified check or cashier's check made payable to the "Four County Landfill Site OU2 Trust". Do not send the check to IDEM. The check, and a transmittal letter accompanying the check, shall reference the name and address of the party making payment, the Site name, and the IDEM Site Identification Number 00000134 and shall be sent to:

- The Effective Date of the Agreed Order is three (3) business days after it was signed by IDEM's Commissioner at the end of the forty-five (45) day period for parties to agree to settle. The Commissioner signed the Order on August 24, 2006. Therefore, the Effective Date, as indicated above, is August 29, 2006. Pursuant to the terms of the Agreed Order you have thirty (30) days from the Effective Date to make your payment if you did not submit it along with your signature page indicating your participation in the settlement.
- Settling Party Appendices. The Appendices below reflect those parties that have settled and made their payments. These Appendices will be updated periodically as settlement payments are received from Respondents.
