Introduction

The Hazardous Waste Biennial report is required by 329 IAC 3.1-7-14, 329 IAC 3.1-9-1, and 329 IAC 3.1-10-1. It is due every even numbered year covering activities conducting during the preceeding odd numbered year. It must be submitted by large quantity generators and treatment, storage, and disposal facilities.

Submittal Requirements

  • For 2007, the Annual Manifest report is required from Small Quantity Generators.
  • For 2007, the Hazardous Waste Biennial Report is required from Large Quantity Generators and Treatment, Storage, and Disposal facilities.
  • Reports must be postmarked by March 1, 2008.

Biennial Report Forms and Instructions

Electronic Reporting

  • If possible all Indiana Biennial Reports should be filed electronically using the American Resource Management (ARM) Easitrak software available at the above link.
  • Please read the Indiana instruction booklet carefully in addition to the Easitrak instructions.
  • Handlers who have their own data systems and can create flat files may submit files in the specified format. Contact Jenny Ranck Dooley for a copy of the requirements.
  • Be sure to indicate clearly when you send your Handler ID form that you filed electronically.

The Hazardous Waste Handler ID form with an original signature must be sent to IDEM in the U.S. mail.

  • Hazardous Waste Handler Identification form (ID form) (signed paper copy)
  • Easitrak Form SI (pages 1 and 4)
  • Easitrak Form GM
  • Easitrak Form OI
  • WM (if applicable) (paper form)

Forms required from TSDs

  • Hazardous Waste Handler Identification form (ID form) (signed paper copy)
  • Easitrak Form SI (pages 1 and 4)
  • Easitrak Form WR
  • Easitrak Form OI
  • CC (paper form)

Paper Forms

If you are unable to use the Easitrak web version of the Biennial report you may still file on paper. You may print out the pdf version of each form, complete the form, and mail it to us. You may also use the fillable form and send it to us in paper or through e-mail.

The Hazardous Waste Handler ID form with an original signature must be sent to IDEM in the U.S. mail.

Fillable Forms

  • MS Word fillable forms allow you to enter your data into a form on the computer.
  • Open the form and save it under a new name.
  • Each page must be saved under a different name, i.e. Company_name_pg1, company_name_pg2, etc.
  • Use the tab key to move from field to field
  • The forms can then be printed and mailed or e-mailed to us.
