The Leaking Underground Storage Tank (LUST) Section is now accepting:

  • Electronic reports, for example:
    • Release Reports - suspected and confirmed.
    • Technical Reports and Plans* - 20-day Abatement Reports, Initial Site Characterization (ISC) Reports, Further Site Investigation (FSI) Reports, Corrective Action Plans (CAPs), Corrective Action Progress Reports (CAPRs), and other reports
      • *one (1) paper copy of all reports, excluding release reports, must always be submitted to IDEM
  • Electronic data, for example:
    • Analytical data, for example: air, soil, or monitoring well data
    • Locational data

Submitting Information Electronically

Electronic reports:

  • Release Reports:
    • Download the Initial Incident Report form
    • E-mail LeakingUST at idem.in.gov within 24 hours of the release
  • Technical Reports and Plans*:
    • Reports must be submitted in a Portable Document Format (PDF)
    • Submit to LeakingUST@idem.in.gov
    • Please include the facility identification (FID) number, LUST number (if available), and Project Manager name (if known) on the subject line of the email.
      • *one (1) paper copy of all reports, excluding release reports, must always be submitted to IDEM

Electronic Data:

  • Analytical Data and Locational Data:
    • The proper format for this information is available on the OLQ Electronic Data Submittal Guidelines page.
    • Submit properly formatted data to olqdata at idem.in.gov.
