Standard Letters to Utilities
- Initial Notice of Proposed Improvement - Blank
Use this form letter to fulfill the requirements of 105 IAC 13-3-1(c). This letter is used to notify a utility of a proposed improvement project. This letter is sent prior to the survey being completed. This letter includes a request for the utility to describe their facilities within the geographical limits of the improvement project.
Initial Notice of Proposed Improvement sample - Verification of Existing Facilities - Blank
Use this form letter to fulfill the requirements of 105 IAC 13-3-2(a). This letter is used to send the utility a copy of plan sheets that show the location of existing facilities as recorded from the survey or from information obtained from the initial notice letter. This letter includes a request for the utility to verify the accuracy of the information presented for their facilities.
Verification of Existing Facilities sample - Preliminary Plans for Improvement - Blank
Use this form letter to fulfill the requirements of 105 IAC 13-3-3(a). This letter is used to send to the utility a copy of the preliminary project plans. This letter includes the request for the utility to declare any conflicts between their facilities and the proposed improvement project. This letter gives the utility a chance to work with a designer on design changes that can eliminate avoidable utility relocations. It is not a request for a work plan from the utility.

Use this form letter to fulfill the requirements of 105 IAC 13-3-3(b). This letter is used to send to the utility the preliminary final project plans. This letter includes the request for the utility to prepare a facility relocation work plan.

Use this form letter to fulfill the requirements of 105 IAC 13-3-3(h). This letter is used to notify the utility their final work plan is approved and to give them notice to proceed with the proposed work.

Use this letter to fulfill the requirements of 105 IAC 13-3-4(b). This letter is used by the INDOT contractor to notify the utility representative when work requested in their utility work plan is completed by the contractor.

Use this form to fulfill the requirements of 105 IAC 13-3-3(c). This form is used by the utility company to prepare a utility relocation work plan. The utilities relocation plans, cost estimate and easement documentation are submitted along with this form in order to meet the requirements of section 8, 9 and 10 respectively if needed.
