Utility Relocation Work Plan - Example
Use this form to fulfill the requirements of 105 IAC 13. This form is used by the utility to prepare their utility relocation work plan. The utility relocation work plan consists of four parts, the narrative, the drawing, the cost estimate (if applicable), and easement documents (if applicable). The drawing shall show what is being removed from service and what is being installed including a description, station, offset and elevation. The cost estimate shall be detailed and conform to the requirements of the FHWA Program Guide.
