This page presents information related to utility coordination for highway improvement projects. This information is designed to support the coordination process required to follow the rules established under Title 105 Indiana Administrative Code 13 “Utility Facility Relocations on Construction Contracts.” The page may be most useful to employees of utilities, consultants, contractors and INDOT.

Work Plans

  • Utility Relocation Work Plan - Blank
    Use this form to fulfill the requirements of 105 IAC 13-3-3(c). This form is used by the utility company to prepare a utility relocation work plan. The utilities relocation plans, cost estimate and easement documentation are submitted along with this form in order to meet the requirements of section 8, 9 and 10 respectively if needed.

Utility Authorized Representatives Information & Address List

To Submit or Change a Utility Company's Authorized Representative Information
This link is for use by a utility company to submit, change or update their authorized representative information.

The rest of the information within this section is intended solely to assist in finding a utility contact person for long range coordination of utility relocation work on public-works highway projects. There is no assurance that the information is complete or accurate.

The Utility “Authorized Representative” contact information list requires a username and password to access.  You must request the username/password by using this link.
