The Home Health Aide Registration Program ensures that home health aides meet the qualifications for a home health aide and are appropriately registered on the Aide Registry. The program administers the Aide Registry.
Effective January 14, 2008 home health aides may register through an on-line process. Learn more about this on-line registration system

Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.
Report an incident regarding a health care facility
The Incident Reporting Form is only for facilities to notify the Indiana State Department of Health of an unusual occurence. This form is not to be used to file a complaint.

Checking the Registration Status of a Home Health Aide
The Aide Registry allows anyone to check registration status of a home health aide for no charge. Only the name of the aide is necessary for checking the Aide Registry.
