Program Overview
The Home Health Aide Registration Program ensures that home health aides meet the qualifications for a home health aide and are appropriately registered on the Aide Registry. The program administers the Aide Registry.

Report a complaint regarding a health care facility
Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.
Report an incident regarding a health care facility
The Incident Reporting Form is only for facilities to notify the Indiana State Department of Health of an unusual occurence. This form is not to be used to file a complaint.

Checking the Registration Status of a Home Health Aide
The Aide Registry allows anyone to check registration status of a home health aide for no charge. Only the name of the aide is necessary for checking the Aide Registry.

This form allows a home health aide to apply for registration by paper.
Home Health Aide Registration Renewal Application
This form allows a home health aide to renew their registration by paper.
On-line Home Health Aide Registration
Home Health aides may register through the on-line process which takes only a few minutes.
