End Stage Renal Disease Facility (ESRD) Certification Program
Program Overview
The End Stage Renal Disease Facility Certification Program monitors dialysis clinics for quality, safe dialysis services for all citizens in need of these services. Dialysis is indicated for people whose kidney function is below 15%. The Indiana State Department of Health inspects End Stage Renal Disease Facilities on behalf of the Centers for Medicare and Medicaid Services.

Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.
Report an incident regarding a health care facility
The Incident Reporting Form is only for facilities to notify the Indiana State Department of Health of an unusual occurence. This form is not to be used to file a complaint.
