Program Overview

An ambulatory surgery center is a health care facility that specializes in providing surgery, pain management and certain diagnostic (e.g., colonoscopy) services in an outpatient setting. In simple terms ambulatory surgery center procedures are more intensive than that done in the average doctors office but not so intensive as to require a hospital stay.

Any facility that meets the definition of an ambulatory outpatient surgical center found in Indiana Code 16-18-2-14 must be licensed by the Indiana State Department of Health (ISDH) on an annual basis. The ISDH monitors the health care quality of ambulatory surgery centers through the licensure process.  ISDH surveyors visit centers for licensure inspections.

Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.

Report an incident regarding a health care facility
The Incident Reporting Form is only for facilities to notify the Indiana State Department of Health of an unusual occurence. This form is not to be used to file a complaint.
