Certified copies of birth and death certificates are available from the Indiana State Department of Health (ISDH) by mail with payment of check or money order. If you prefer to order on-line and pay using a credit card, you may do so through the Vital Records Online Ordering System.

You may contact the local health department in the county where the event occurred to obtain information on their walk up service hours, method of acceptable payment and if they accept order via the internet. All requests require proper identification and proof of relationship to the person whose record is being requested is required. Methods of payment and fees may vary. See requirements.

Birth records in the ISDH Vital Records office begin with October 1907. Prior to October 1907, records of birth are filed only with the local health department in the county where the birth actually occurred.

Death records in the ISDH Vital Records office begin with 1900. Prior to 1900, records of death are filed only with the local health department in the county where the death actually occurred. For deaths occurring from 1900 to 1917, the city and/or county of death is required in order to locate the record.

To apply for a birth certificate by mail and pay by check or money order:

Please print form 49607 . The form includes applicable fees, identification requirements, and mailing address. The cost for the first certificate is $10.00 and $4.00 for each additional copy. Please make check payable to Indiana State Department of Health.(Average Processing Time: 3 to 4 weeks. Processing time begins the day the ISDH Vital Records office receives the request.)

Please print form 49606. The form includes applicable fees, identification requirements, and mailing address. The cost for the first certificate is $8.00 and $4.00 for each additional copy. Please make check payable to Indiana State Department of Health. (Average Processing Time: 3 to 4 weeks. Processing time begins the day the ISDH Vital Records office receives the request.)

This training module is a collaboration between Vital Records and the Maternal and Child Health Divisions at the Indiana State Department of Health. It is the first of many more trainings on a variety of topics. Our target audience includes nurses, birthing center staff, physicians, hospital administrators, supervisors, midwives and local health department registrars.

This module focuses on: The importance of collecting quality data on the Certificate of Live Birth, what the federal government and ISDH do with the data, and what resources are available to assist you in the collection of the data. We hope that in understanding the uses and resources available to collect the birth data, the quality and accuracy of Indiana data will improve.
