Program Overview

The Intermediate Care Facility for the Mentally Retarded (ICF/MR) Program enforces rules and regulations for over 500 facilities.  The majority of these facilities are four to eight bed group homes.  Staff conducts certification surveys and investigates complaints. The surveys focus on observations of staff and/or consumers interaction and interviews with consumers regarding their participation and choice of services in order to determine the outcomes of active treatment.

Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.

Report an incident regarding a health care facility
The Incident Reporting Form is only for facilities to notify the Indiana State Department of Health of an unusual occurence. This form is not to be used to file a complaint.
