Program Overview

The Comprehensive Care Facility Licensing and Certification Program enforces the rules and regulations for long term care facilities. The Program staff conducts health and life safety code surveys and investigates complaints in order to assess the compliance of the facilities with the rules and regulations.

Save the Date!

The Indiana State Department of Health (ISDH) invites all comprehensive long term care facilities to attend the Indiana Long Term Care Leadership Conference scheduled for March 24, 2009 in Indianapolis at the Indiana Convention Center. The topic of the March Leadership Conference is “Incontinence.”  The ISDH invites you to send two representatives to the conference.  Each facility may select the individuals attending the conference on behalf of their facility. The ISDH recommends individuals such as the facility administrator, director of nursing, or compliance officer attend this conference.  Corporate offices of facilities are also invited to send appropriate representatives.

Conference Brochures and Online Registration are not available at this time.

Report a complaint regarding a health care facility
Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.

Report an incident regarding a health care facility
The Incident Reporting Form is only for facilities to notify the Indiana State Department of Health of an unusual occurence. This form is not to be used to file a complaint.
