How to submit a face by mail:
Send your photograph and donation to the Indiana State Museum Foundation. Your original photograph will be returned to you. Be sure to include:
- One photograph (maximum size 8" x 17"; originals will be returned; no photocopies, please)

Name of family or group in the photograph
- Approximate year the photo was taken
- City where photo was taken or where the family lives/lived
- Description of the photo and its subject matter (50 words maximum)

Your name, address and phone number
- Your e-mail address (optional)

A $100 contribution made payable to the Indiana State Museum Foundation
