Name Change
Name changes must be made, in writing, to the Board of Law Examiners. Some proof of the legal change must accompany the request. Please mail or fax any name changes to the Board of Law Examiners Office.

You may change your address with the Board of Law Examiners by U.S. mail, email, or by using the following online form. See the contact page for U.S. mail and email information; otherwise, please submit the form below.
Note: All fields in this form are required. If you do not complete all fields, your address change will not be submitted to the Board of Law Examiners.
YOU SHOULD ONLY USE THIS FORM IF YOU HAVE ALREADY SUBMITTED AN APPLICATION FOR FOREIGN LICENSE OR AN APPLICATION FOR BUSINESS COUNSEL LICENSE AND YOUR ADDRESS HAS CHANGED SINCE THAT APPLICATION WAS SUBMITTED. |
