Instructions for Certification of New Officers, Directors Shareholders, Members, Partners, Other Equity Owners, and Lawyer Employees
(only complete if your firm has hired new attorneys):
- Enclose, a fee of $10.00, per attorney new to the firm, payable to the State Board of Law Examiners. Do not include non-Indiana attorneys or attorneys already certified. This is the only fee that may be combined with the $50.00 Renewal Fee.
- File with the Board of Law Examiners (BLE), an original and one (1) copy of Indiana Supreme Court Disciplinary Commission certification form
that no disciplinary action is pending against any officers, directors, shareholders, members, partners, other equity owners, or lawyer employees licensed in Indiana. Attach a list of new attorneys to be certified. BLE will forward this to the Disciplinary Commission. There is no fee for this portion of the certification.
- File with the Board of Law Examiners (BLE), an original and one (1) copy of Indiana Supreme Court Clerk's certification form
that each of the officers, directors, shareholders, members, partners, other equity owners, or lawyer employees required to be licensed in Indiana are licensed to practice law in Indiana. A fee of $1.00, must accompany this for each attorney new to the firm, who is licensed in Indiana and needs to be certified. This check must be separate of any other fees and must be payable to the Clerk of the Indiana Supreme Court. BLE will forward this form and fee to the Clerk’s Office.
The attached listing (Sample Listing: MS Word | Adobe PDF ) must include only the officers, directors, shareholders, members, partners, other equity owners, and lawyer employees, licensed in Indiana, who need to be certified - no others. See attached sample listing for formatting. Do not list non-Indiana attorneys. Return the completed forms with attached listings to the State Board of Law Examiners.
