***The public hearing on the Continuing Competency Rule scheduled on December 6, 2012 before the Indiana Medical Licensing Board has been moved from Conference Room 22 of the Indiana Government Center South.  The hearing will be held at 1:30 p.m.,  Local Time, in Room W064 of the Indiana Government Center South, 402 W. Washington Street, Indianapolis, Indiana 46204***

 Effective July 1, 2011 all PT and PTA applicants must submit a criminal background check (CBC) prior to being licensed; any applicant seeking a temporary permit will be required to submit a CBC prior to the temporary permit being issued.  For more information and instructions on the CBC click here.
