Pre-Employment Testing Consent Form
This form is for a candidate to sign authorizing a pre-employment test. It also indicates his/her acceptance that if he/she tests any thing other than negative, the employee cannot be employed.

Use forms to obtain information from the employee's prior employers for the previous two years. You must document your attempts to obtain this information.
Add/Delete Form
This is an essential form. Management must send this form to Midwest Toxicology Services and the State Personnel Department - Employee Relations Division whenever a CDL employee is hired or leaves service. This form must be sent as soon as possible so the random pool is up-to-date. Also available as a PDF.
Certificate of Receipt
This form may be used to indicate an employee has received all appropriate education materials for the CDL program. The signed receipt must be maintained in a location that can be forwarded to SPD within a day.
Off-Site Collection Form (necessary for off site collections)
This is another essential form. If management sends an employee to a collection site for an alcohol and/or controlled substance test, management must complete this form and send it with the employee to the collection site. The form is essential because it routes the sample to the correct laboratory for processing and bills Midwest Toxicology Services.
Observed Behavior/Reasonable Suspicion Record
This form is available if management wishes to use it to document behaviors for a reasonable suspicion test. Management must document in writing observed behaviors that warranted the test.
Employee Authroization to Release Test Results
This form is for an employee to complete if he/she wants a copy of his/her test results under the State's program. This is not the form to use to obtain information from prior employers. Employees commonly complete this form so their designated union representative may obtain a copy of their test results.
