2008 One Care Street Enrollment

Over the spring and summer months, state employees will be receiving letters from One Care Street, encouraging them to sign up for this valuable benefit. To determine when you should expect to receive your letters, view the 2008 OCS deployment schedule.

Make sure to complete your survey by the deadline, as there will be no extensions. If you miss the deadline, you will not be able to participate in the One Care Street program. Not only will you miss out on the financial incentive ($260 for those with single coverage; $390 for family coverage), but you will also lose out on opportunities to have professional health coaching, guidance and information available to you on health and healthy living issues.

Q: Who is eligible to participate?
A: All State of Indiana employees and their spouses that are covered under the State's medical insurance.

Q: What kinds of questions are on the One Care Street Health Profile?
A: Questions about physical symptoms, medical conditions, levels of stress emotions, lifestyle behaviors, levels of care use from the past six months, healthcare beliefs and preferences, etc.
